Key Highlights of the FDA Announcement
- The FDA is requesting drug manufacturers update labels on menopausal hormone therapies to remove broad boxed warnings tied to cardiovascular disease, breast cancer and probable dementia risk.
- The change follows a comprehensive literature review, an expert panel convened in July 2025, and public input on the balance of benefits versus risks of hormone replacement therapy (HRT).
- Removal of the recommendation to prescribe hormone therapy at the lowest effective dose for the shortest duration—treatment decisions are individualized and fall within the clinical judgement of a clinician in discussion with a patient.
- The updated guidance places greater emphasis on timing of initiation: women who start hormone therapy within 10 years of menopause onset (or before age 60) show markedly improved benefit-risk profiles.
- While some warnings remain (for example, endometrial cancer risk where estrogen is unopposed in women with a uterus), the shift opens greater flexibility in therapeutic decision-making.
